People with disabilities, chronic conditions, and seniors, along with their families, often need help securing and maintaining stable housing. Support is available in three forms: temporary housing, continuous assistance, and advisory services.
Developing individualized plans tailored to housing goals, necessities, and aspirations.
Assisting clients in making well-informed choices regarding housing options.
Providing support and resources for non-housing needs.
Partnering with other organizations and agencies.
Helping individuals identify their ideal housing options.
Providing support through the housing search and application process.
Creating a financial plan.
Understanding rental contracts.
Educating tenants about their rights and obligations.
Providing strategies for fostering positive relationships with landlords and neighbors.
Teaching responsible tenancy practices and ensuring adherence to lease agreements.
Assisting in resolving issues to maintain stable housing.
Must be on Medical Assistance (MA).
Must be 18 years old or older.
Should have a documented disability or disabling condition.
Must be assessed to require assistance resulting from the presence of a disability or a long-term or indefinite condition.
Must be experiencing housing instability or housing distress.
At risk of homelessness.
